Point of view (POV)
Whose consciousness the narration is filtered through in a given scene.
POV is the single most common thing to break in a long draft, and the hardest to notice while writing. A close third-person scene slips into knowing what another character is thinking; a first-person narrator reports something they could not have seen.
It breaks more often with AI assistance, not less: a model given a scene without an explicit POV will default to whatever the surrounding prose implies, and "implies" is doing a lot of work in a scene with three people in the room.

POV and tense are set per book and can be overridden per scene, and both travel into the generation context. A flashback written in a different tense stays in that tense instead of drifting back.
